By submitting a card request to Jayden’sCards4Cancer, you agree to the following.

• This request must be submitted by a parent, legal guardian, or other authorized person.
• The child must currently be undergoing cancer treatment or must have previously undergone cancer treatment.
• Cards are subject to availability. Specific cards, players, teams, brands, or quantities cannot be guaranteed.
• Jayden’sCards4Cancer may contact the requesting family for additional information needed to fulfill the request.
• Participation in photo, video, or story sharing is voluntary and is never required to receive cards.
• Public use of any submitted media requires the applicable Media and Story Permission agreement.
